The required application package includes: (1) a proposal form that clearly describes the organization’s background and a detailed project description; (2) the federal assistance application form, using SF-424 for organizations or SF-424i for individuals; (3) a detailed budget using SF-424A and SF-424B along with a budget narrative; (4) for organizations, proof of active online registrations in DUNS, NCAGE, SAM.gov, and Grants.gov, noting that registrations must be updated annually and that legitimate registrations do not require fees; (5) a key contact form and CVs for relevant personnel; (6) the required certification documents; and (7) the SF-LLL Disclosure of Lobbying Activities form.
